What is the Support at Home program?
Support at Home is the Australian Government’s in-home aged care program, replacing the Home Care Packages (HCP) Program and Short-Term Restorative Care (STRC). It is designed to help older Australians remain independent by providing personalised care and support in the comfort of their own homes.

Start by applying through My Aged Care on 1800 200 422 or visiting myagedcare.gov.au . An assessor will then arrange an assessment through the Single Assessment System. Once your funding classification is approved, you can choose Living Waters Aged Care as your registered provider.
Can I switch to Living Waters Aged Care from another provider?
Yes. Let your current provider know that you wish to change providers and confirm any notice period or exit conditions. You can then contact My Aged Care to activate your referral code. Once you have your referral code and confirmed exit date, our team can help guide you through the transfer process.
